View Additional Documents
All documents your provider has generated & released to the Client Portal will appear below. This information may not apply to every client situation. For questions, speak with your provider.
Documents may include: General Note, Good Faith Estimate (GFE), Evaluation, and Progress & Discharge Notes.
View & Transfer Documents
Section enables the upload of external documents & transfers documents to your provider.
Note: This is an optional section of Embark EMR, and your provider may have deemed this feature unnecessary.
Generate Invoice
All provided services with an outstanding payment will display on the below document. First select the applicable date range (leave blank for all dates), then click ‘Search’. The Invoice automatically displays client information. To update information not present, update via the ‘Client Information’ tab or speak with your provider.
Convert the generated document to a PDF by clicking the “Download Data” link just above the generated table. This will reformat the data on the page to fit on a PDF, and appear in your downloads folder when complete.
Generate Receipt
All provided services with a payment applied will display on the below document. First select the applicable date range (leave blank for all dates), then click ‘Search’. The Receipt automatically displays client information. To update information not present, update via the ‘Client Information’ tab or speak with your provider.
